GENERAL SCOPE OF SERVICES:
INFORMATION REQUIRED FOR RFQ RESPONSE:
The evaluation criteria to be considered for the Request for Qualifications are listed below. An award will be made to the responsible offer or whose RFQ response is determined to be the most advantageous to the City of Mt. Juliet. No other factors or criteria will be used in the evaluation. The response to the RFQ shall address the following criteria and be submitted in this order:
1. Name of the contracting firm with the following information: owner; address; and telephone number. A listing of all proposed sub-consultants with the following information: name of project contact; address; telephone number.
2.Corporate Experience, Qualifications, and Project References.
Provide information about your firm’s and personnel’s experience in completing the following tasks relative to a Municipal Police Department Facility: development of building layout drawings with elevations; programming and planning; project management and coordination; Preparation of Design/Construction Drawings; Preparation of project specifications; Preparation of bid and contract documents; preparation of estimates of probable costs on projects of similar size and scope; assisting with the evaluation of bids; and making a recommendation for awarding a contract for construction.
3.Identification of Project Team Members assigned to this project with a listing of relevant project experience for each.
Identify the members of the Project Team (lead consultant and all proposed sub-consultants) and the role to be played by each member of the Team. Provide a proposed team organizational structure showing interrelationships and interactions. Provide resumes of all personnel who will be involved in providing the required services with relevant experience and professional certification/registration of each.
4.Capacity to perform the work in a timely manner.
Provide information which documents your firm’s and sub-consultants’ qualifications to produce the required services and service levels.
5.Understanding of the Project.
Detail your understanding of the project scope and desired deliverables. Provide a detailed plan of your firm’s proposed approach, including major tasks and sub-tasks. Include a proposed quality assurance plan your firm will conduct to ensure the performance of contract deliverables.
6.Conflict of Interest.
The firm, by submitting a proposal, certifies that to the best of its knowledge or belief, no elected or appointed official of the City of Mt. Juliet is financially interested, directly, or indirectly, in the purchase of services as described in this RFQ.
